Wirt, form has personal history information headings printed on it in black and answers handwritten in pencil, top of the form reads "STUDENT'S HISTORY BLANK.", handwritten top right in black marker is "35" over "119" in black ink, two hole punches in the top edge; 13.8 cm (w) x 21.5 cm (h). Paper yellowing, corners worn, both right corners bent, top bottom and right edges bent, top and left edges torn, some bending throughout the page, bending and tearing around the hole punches, brown spots and stains, black marks and smudges, red-brown stain on the back top edge, red plastic on the top edge from where it was attached to a pad. |
