Student history form filled out for Dora May Walrod, form has personal history information headings printed on it in black and answers handwritten in black ink, top of the form reads "STUDENT'S HISTORY BLANK.", handwritten top right in black marker is "38" over "226" in black ink, two hole punches in the top edge; 14 cm (w) x 21.6 cm (h). Paper yellowing, corners worn, all corners bent, all edges bent, top edge torn, some bending throughout the page, bending and tearing around the hole punches, brown spots and stains, black marks and smudges. |
