Student history form filled out for Benton Fuller Gentry, form has personal history information headings printed on it in black and answers handwritten in pencil, top of the form reads "STUDENT'S HISTORY BLANK.", handwritten above the title in black ink is "F-13", "66" handwritten top right in black ink, large amount of information written at the bottom and then erased, two hole punches in the top edge; 14 cm (w) x 21.5 cm (h). Paper yellowing, corners worn, bottom left corner bent, some bending throughout the page, bending and tearing around the hole punches. |
